To request and appointment, call EmPower Treatment Center at 507.292.1379.
At your appointment, the Clinical Specialists will help you fill out some brief paperwork. Then you will meet with your chosen Clinician to discuss your needs and develop a treatment plan.
New Client Forms
Fill out appropriate forms and the Clinical Specialist will have a file started to for you to make your appointment as effective as possible. Bring Photo ID, Insurance card(s) and a list of current medications and any referral forms as well.

Billing & insurance is handled by LM Billing
We have Care Coordinator on staff to help with resources.
